Nurse Practitioner Opportunity – Hospice

Centra Medical Group, located in beautiful Lynchburg, VA, has an opportunity available for a Nurse Practitioner to work with our Hospice program. 

Our ideal candidate is an NP with hospice experience.  We will consider new grads with a strong interest and experience in end-of-life care.

Responsibilities include physical assessments, care plans, inter-disciplinary team meetings, and working with medical directors and nurses in providing acute home and facility visits to meet regulatory requirements and to decrease emergency room visits and hospitalizations.   Education on-end-of life, medication management, wound care, etc to staff and families.  Opportunity to participate/create/lead innovative projects that advance quality care of the patients.
