The Home-Based AIM Nurse Practitioner provides specialty palliative care in the home setting for adults with advanced, life-limiting illness and their families. The Nurse Practitioner collaborates with the AIM team (social worker, chaplain, RN Coordinator, and Medical Director), and the patient’s primary care provider and specialists to coordinate care and effectively meet the medical, psychological and spiritual needs of the patient. This enables patients to live life as fully and comfortably as possible in an atmosphere where patients and families are able to share their thoughts, hopes and concerns. The Nurse Practitioner is responsible for assessment, establishing goals of care, ordering tests, treatments and therapies for patients, and prescribing medications as delegated and licensed. The Nurse Practitioner identifies and addresses gaps in care and provides teaching and disease management to patients as needed.
